✅ Use a fasting serum creatinine: Dietary protein intake can temporarily raise creatinine. For the most accurate result, use a fasting serum creatinine sample taken in the morning.
✅ IBW for obese patients: When actual body weight exceeds IBW by more than 20%, use IBW or Adjusted Body Weight (ABW = IBW + 0.4 × (Actual – IBW)) to avoid overestimating CrCl.
✅ Elderly patients: Despite apparently normal serum creatinine, elderly individuals may have significantly reduced kidney function due to decreased muscle mass. Always account for age carefully.
